Back in July we announced the launch of an innovative health care trial that we believed could revolutionise the way people are cared for in the Illawarra. The initial focus of the trial was on early discharge care packages through hospitals and was subsequently expanded to include health assessments by Registered Nurses as well. The objective of the health assessment was to build awareness of appropriate community services that members may be eligible for and to ensure they are aware of how to access hospital substitute care packages for any future admissions they may have.
The overall feedback from participants has been very positive with most individuals supporting the use of the program in the future. In addition client health outcomes were positive and the risk of re-hospitalisation was minimised.
Survey results show that 86% of participants surveyed felt the nurse helped them maintain their independence at home, and 100% felt the nurse helped improve their confidence in managing their health. When asked what they thought was most useful about the program, members reported:

When considering their future needs, 86% of members surveyed felt it was likely they would use the hospital substitution program in the future.
