Hospital Substitute Services
A coordinated, targeted approach to care which includes a mix of assessment/triage, hands-on services, and assistive technologies.
Hospital Substitute Services are for those seeking early discharge, or who are at risk of hospitalisation or readmission. It’s also for those with chronic or complex care, acute episodes or conditions requiring close monitoring or coordination.
The Process:

Care plans are developed by your doctor in consultation with you.

A Registered Nurse from INS conduts a comprehensive assessment to identify OH&S / falls risks; dust and mildew areas; beds, toilets, or chairs that are too high or too low; anxiety / depression, etc.

Consumables (such as continence or would care supplies) and equipment (such as walking frames or shower chairs) are provided by INS Health Care

Referrald to community support services, as appropriate, e.g. physiotherapist, occupational therapist, podiatrist, dietician, HACC, aged care assessment team, and allied health professionals.

Your health is monitored at each visit by the RN who inputs data into our LifeGuard Intuito innovative workflow management system.

Reports on your recovery can be transmitted electronically to the doctor during your recovery period along with clinical measurements and wound care photos.

You may also be provided with an emergency button that can be used 24 hours a day, seven days per week to access nursing staff at the INS LifeGuard Call Centre.
