INS LifeGuard Now a Coordinated Veterans’ Care (CVC) Provider
The Department of Veterans’ Affairs has invited INS Health Care to participate in the Coordinated Veterans’ Care (CVC) Program, which commenced on 1 May 2011. The CVC program intends to reduce preventable hospital admissions and readmissions through better community-based coordination of care.
The Program is for eligible veterans, war widows/widowers and dependents who are gold cardholders with chronic conditions, complex care needs, and who are at risk of hospitalisation. The Program commenced on 1 May 2011.
Through improved community-based care, the Program is intended to improve the health of participants by:
- coordinating care in conjunction with their GP
- coordinating appointments and feedback with other health professionals involved in the care of the veteran or war widow/er
- undertaking home visits and assessments
- providing appointment reminders
- assisting with medication and treatments
- educating and empowering participants to self-manage their conditions
- encouraging the most socially isolated to participate in community activities

INS LifeGuard accepts DVA cards to help eligible veterans, war widowed partners and dependants access personal alarm solutions that can aid them in living safely and independently.
